Request an Appointment

You can request a medical or dental appointment online. Please fill out the form below with your name, phone number and email address along with the problem you are having. Instead of the form, patients can call 580-354-9007 and leave a message.

The clinics are open from 5 p.m. to 8:30 p.m. or until the last patient leaves. The first and third Thursdays of each month are for medical appointments. The dental clinic appointments are split into two days. The two days consist of a Tuesday screening and X-ray on the same week as the second Thursday of the month, which is the extraction day.

Medical Patients, 18 and older, requesting a medical appointment cannot have any medical insurance, earn 200 percent or less of Federal Poverty Level and if you are an Oklahoma resident, a denial letter from Soonercare/Medicaid.

Dental Patients, 18 and older, requesting a dental appointment must make an appointment at the medical clinic if they meet all the criteria for medical patients (above). The doctor will refer them to the dentist. Exception: If you are seeking a dental appointment, have medical-only insurance and meet the low-income requirement, you can request an appointment with the dental clinic by filling out the form or by calling 580-354-9007 and leaving a message.

Hearts That Care does not offer a vision clinic yet. The vision clinic will open this fall or in 2023.

Note: Uninsured patients must make an appointment with our medical clinic before being seen in our dental clinic, any specialty clinic and our future vision clinic before being seen in any of those clinics.

To see you are eligible for SoonerCare/Medicaid or to get a denial letter, call 580-585-6746 during business hours for a 15- to 20-minutes phone call. If denied Soonercare/Medicaid, ask the representative to send the letter to your email account or to print it out. You can pick it up during business hours at the Comanche County Health Department, 1010 S. Sheridan Road, Lawton, OK 73505. If approved, you will be given a SoonerCare member number. If denied, please call our clinic for an appointment.
