Is there an income requirement?

Yes, less than 200% poverty level.

For families with more than 8 persons, add $4,180 for each additional person to determine poverty guideline.

What is proof of income?

Federal tax returns from the previous tax year–page 1 and 2 from your 1040 form. We can make copies at the clinic if needed. If you did not file income tax, we need to have your last 2 pay check stubs. If you live with someone, we need their tax forms. (living with family or friends). We use this information to get medications for you free of charge; we cannot do this service without income information. Helpful information is a rejection letter from Food Stamps–you need to go to DHS on Lee Blvd to get that form. If you have social security disability, you need to apply for Medicaid and if you do not qualify, we need the form they give you there.

What if I make more than 200% of the poverty level?

There is a clinic in Lawton that operates on an income sliding scale and is open during business hours:

Do I need to live in Lawton?

We will see anyone who does not live in a community that already has a volunteer health care center. There is a center in Duncan and Altus. If you live in those communities, please go to those clinics.
